• It will be your responsibility to pay your co-pay upon each visit. Any outstanding balance not covered by insurance must be addressed upon each visit to the office. 
• Please bring your current insurance policy card and proper identification with you to every appointment 
• If you are a new patient, please make sure to bring photo identification and any additional required documents requested upon booking your appointment

Please click the “Book Now” button to request and schedule an appointment. Once you have scheduled your appointment with Dr. Adeogun, you will be asked to complete a series of forms. It is important that you complete them in their entirety. Upon booking, you will be provided with a list of all necessary paperwork. Please be sure that you have made arrangement with your health insurance provider and are prepared to pay the appropriate charges
